Verified healthcare data breach — disclosed June 2026. Family Health Centers of Southern Indiana (FHCSI), a community health provider operating multiple locations and a mobile dental unit in southern Indiana, became aware of unauthorized network activity on or about January 16, 2026. Forensic review found evidence that certain files may have been accessed by an unauthorized party. FHCSI completed its investigation and began notifying affected individuals June 18, 2026, posting a substitute notice June 22, 2026. The U.S. HHS OCR breach portal lists Family Health Centers of Southern Indiana with 7,117 individuals affected, submitted June 18, 2026, as a hacking/IT incident on a network server. FHCSI is offering credit monitoring for individuals whose SSNs and financial information were potentially impacted.
