Forensics determined access occurred between November 16 and December 16, 2025, with emails and files potentially viewed or acquired. A February 17, 2026 file review confirmed exposure of extensive PHI and identifiers—including SSNs, driver’s licenses, Medicaid/Medicare numbers, financial account data, diagnoses, prescriptions, and medical record numbers. Notifications were sent April 17, 2026, with complimentary identity protection services offered.
